Insert new field in table "Item Substitution&q

Hello everybody,
I've got a probplem with the form "Item Substitution Entries". It doesn't
shows all values from the table "Item Substitution".
I created some new fields in the table 5715 and if I add this fields
due the field menu to the form it doesn't shows the values from the table - but if I run the table I see all values.
Does anybody have expierence with "Item Substitution"?

You need to modify the form and add the fields to the form as well.
form 5716, form 5718, form 5720.
