Run a report from a radio button selection

Hi All,
I'm trying to create a simple form with two radio buttons.
If radio button1 is selected then open Report X.
If radio button2 is selected then option Report Y.
Please keep in mind there are three buttons which are, OK, Cancel, Help.
The Report should run once the radio button in the form is selected and the OK button is pushed.
Can anyone please share the code for me and which trigger that code needs to be added on?
