Can we do Data Upgrade from NAV 5.0 to Business Central?

If yes then please share any guide link or document.

The best way is to move to NAV 2018 first. Cross test everything. If you are satisfied here then gradually start converting your Custom codes to extension and then move.

Business Central you should always plan step by step because there's architectural change.

Considering from where you come, you better consider it a completely new implementation.
You can't just upgrade objects to BC.
