Recently one of our users has been periodically having header code injected at the top of her emails when sending one of our custom reports using the "file/Send/Report by E-mail..." option. What displays is shown below and it is not every time she sends the report which is very puzzling.

I have not been able to find anything online that fits our situation and this is more of a "has anyone else seen this" kind of post. The report has one parameter so the odds of something being done different each time is quite slim. I have not been able to replicate it while using the same process and parameter that she sends each time. Any idea would be greatly appreciated.
The only change recently to the report was adding a text box at the lowest footer that is a static text value.
