NAV 2013: Duplication of MS Office Action Buttons

We've encountered a strange behaviour of Role Centers in a customers database.
For some users the MS Word and Excel-buttons duplicate every once in a while. This leads to an error due to doubled Action IDs.
And this won't stop! They keep duplicating.
First we thought the reason was the Import of a Rolecenter from NAV 2009. But it happens with new Rolecenters as well.
Any idea?
