Qty Per becomes Double on Refreshing Prod. Order

Hi,
In some of my Prod. Orders while refreshing the Qty & Qty Per on Prod. Order Components becomes double. I have checked the BOM and Unit of Measure and they all seems correct.
Anyone have faced this before?
