is it possible to create such a page.
See Appendix. It's about the headings for the different columns.

No this is not possible, the closest i came up with in a recent implementation was this:

(Note that the user has to manually choose the 2-Row Header Option on the page => Right click on header, "Choose Header Height" => 2)
