Multiple choice lookup

Dear All Master
i am a newbie in Microsoft CRM. i have a problem to custom lookup with multiple choice.
i want to have 2 choice entity in my lookup (contact and Account).
i had try a artikel from google http://sureshmscrm2011.blogspot.com/201 ... k-for.html
but i gettting error when load form.
please help me..
