Do you have Cost Amount (Expected) on your Output Item Ledger Entry?
Is there an Invoiced Quantity on your Output Item Ledger Entry?
Is the production order have a status of Finished?
If your version is old enough the output will not invoice until the source of all of the consumption entries are fully invoiced regardless of the costing method.

You should turn on the debugger and see if there is any trigger in items table which changes the value of item's cost

You must change your status RPO to FPO, then you run Adjust Cost Item Entry, and check your cost...
