Patch updating/revamping strategy

Hi all,
Our database is a fully customized one and documentation is not available for most of the customizations. So we are facing difficulty if some new patches or some hot fixes come. It is not possible to just overwrite everything and upload the patch.Also it is really tedious to make out where and all changes should be made by comparing codes in the patch and our database.
Which is the best methodology for updating patches and hotfixes?.
Now we want to revamp the entire system. I can get a standard fresh database from customer source. I want to get every customization in our live database to be documented. How can I do this?. Can anyone please advise on this?
