Item Card Notes Disappearing

NAV/Navision Classic Client
We are using Nav 5.0 and currently have started to notice that some notes that we enter in an Item card are not there the following day. Any ideas as to why this might be occurring? We are using the native DB.

somebody is deleting them. Or it could be code somewhere.
To find out, remove permission for everybody and see who gets the permission error.
