Matrix line limitation?

I'm in the process of creating a Matrix form similar to the Budget Matrix form. The one difference is instead of having just a code and name I need to include code1, code2, code3 and a name. I have data coming it but it just duplicates the same record. If I remove code2 and code3 from the matrix and from the programming it works fine. Any suggestions?
