Dimension Value Lookup to Return Code (not Dimension Code)

I have a Report Request form looking up the Name column in the Dimension Value table.
It is only returning The Dimension Code.

How do you return to the Option's value the Name in stead of the Dimension Value?

On the Report Request Form:
The lookup form's id is set to the Dimension Value's Code Column
The form it is looking up has a TableBox object with Dimension Values (set to Lookup).
