Business Notification Blank Subsription Page

I have installed Business Notification and it seems to work. However when a user is tries subscribe to a BN alert a blank internet explorer page is displayed.
The user opens ie with the adress "http://<servername>/BusinessNotificationSubscribe", a subscribe page should open but it doesn't : a blank page opens.
please can you help me.
