Basically we would have a table with USERID Time and Description of Event. If the User assigned is logged in and the event time is reached it would popup a to do message. If the User was not logged on and an event was reached the next time the user logged in the to do would pop up the overdue message.

This is what the Outlook Sync. was meant to do. You sync your Outlook with the To-Do's in NAV, and Outlook will manage the pop ups for you.

Ignore my advice, Alex's suggestion makes much more sense. For sure using built in functionality is a better way to go.
