How to create menu and menu items on any form of axapta dyna

Hi,

It is require to add menu and menu items on to the specific forms of Axapta dynamically.
1:Whether is it possible?If yes then how?
2:Upon creating the menu items some code in c# is to call from some dll.How to achieve that?

It would be great if anybody could give light on it.
