is it possible on NAV 5 to modify the contextuels menu (exemple add à item )

You cannot modify menus at run time. There are two ways to control menu
1. Create a menu for the context and use assign users
2. use Permission
Another solution is to develop a form similar to pre version 4.0 worked and then hide/disable links
Can you give more background of what you want to do?
