I understand that the FIFO cost is never recorded on the sales lines, it is always the average cost and this cost is carried through from sales order line all the way through to posted invoice line - it is never adjusted. I see that the FIFO cost is reported in the Cost Amount (Actual) flowfield on the Item Ledger Entries.
My question is - what is the best way to report FIFO costs and GP% on a item sale by item sale basis? I think Sales Analysis reports would only give summary by item?
Any help appreciated.

Items can be analysed individually, category wise, or any other way. All it needs is tricky row and column layouts.

Are you calcuating the GP% from the Accounting Cost or the True Cost?

My client is focused on the cost per sales transaction and I'm writing a report based on the Cost Amount (Actual) from the Item Ledger Entry table, so I guess I'm looking at the 'True Cost'.

In your case, you're on the right track. However, most sales reports in standard NAV operate off of the accounting cost, which is not what you need.
You may want to look at maybe adding the GP% on the Item Transaction report or create a sales report off of the Item ledger.
