Multimodal dangerous goods form (report)

Has anyone made a report that fills a multimodal dangerous goods form?
It seems to me that this is a standard report and I wondered if I could have it.

Are you talking about the MSDS forms? The MSDS is required for flammable material typically used with companies dealing with chemicals. It may be different per country.

However, Lanham's E-Ship module has a pre-formated Hazardous form available.
