Import Dataport

I created a Dataport to import Frozen Prices. I only have Item No, and List Price on the Text file.
The rest of the information I am getting it on the Request Form. (Beginning Data, Ending Date, Requested by, etc). The problem is only Item No. and List are getting import to the Frozen-Special Table.
Here is my code:

Without more data I'm guessing, but it looks like you have FrozenSpecial declared as a variable separate from the dataport Data Item, then you have only the 2 variables importing to the Data Item, then you are filling out FrozenSpecial but there is no INSERT.0
