The workflow step instance does not exist?

in NAV Three Tier
I have added workflows to a new table (custom) and it is working fine. I am getting this error from time to time and wondered if anyone else had come across it. What is the best way to fix the system and get it on its way?

    Re-created the test environment. Problem disappeared, I think I have been messing with the table too much :)

    This only seems to occur if I restart the service
