Posted sales invoice and shipment is not created on post

Intermittently and without visible error message, sales orders have been posted but a Posted Shipment and Posted Invoice is not created. Info exists in value entries, item ledger entry, G/L Entry. I am very confident that these posted documents are not being manually deleted. Has anyone ever experienced this? and if so how do i keep it from happening again. Version 4.0 SP3

    Take a look to customisation. This newer happens in standard, AFAIK

    Yes this implementation has been heavily customized. But the odd thing is that the CU 80 was last modified Dec 2008 and these issues only started happening around 05-2012. If it is a customization problem, i would have thought that it would be happening more frequently and sooner.

    maybe some setup? Like check box somewhere in sales setup?
