address in sales order and invoice

I found in our current system that when it's foreign countries outside US, the state field can never be printed out in the sales order and sales invoice form. Even though I can change the report code based on the each of the retrieved field, but the format is not good. Is it the only way to fix the code in the code unit? Where else? Thanks in advance!

I have all My countries setup as

City+State+ZIP Code

Regardless

I tried the link for the list of countries and the appropriate address format and it is a bad link. Is there anyone out there who ships internationally who is willing to share their setup table?
